Providence Partners is a referral and marketing network for qualified adult foster care homes, assisted living operators, and approved care providers with available rooms.
Stress Free Aging helps families find appropriate care. When a family’s needs match what your home provides, we can introduce them to you and help coordinate the placement process.
For qualifying hospice transition placements, participating providers may receive a rate of $250 per day, up to $5,000 within a 30-day period. Providence Partners members pay no placement fee on these qualifying hospice referrals.
You provide the welcoming space and dependable care. We help bring families to your door.

Participation, referrals, and placement payments are subject to program requirements, availability, licensing, care needs, and final approval.

Can anyone with an extra bedroom participate?
Providence Partners is currently presented as a program for AFC homes, assisted living operators, and other approved care providers. Eligibility should be confirmed before an application is accepted.
Is the $250 per day guaranteed?
No. The rate applies to qualifying hospice transition placements. Participation in the network does not guarantee a referral, placement, occupancy, or income.
Does every referral have no placement fee?
No. The zero-placement-fee benefit applies only to qualifying hospice transition referrals. Standard placement fees apply to regular, non-hospice placements.
Does joining guarantee that my open bed will be filled?
No. Stress Free Aging markets participating providers and makes appropriate introductions, but placement depends on family needs, availability, location, budget, and whether the provider is a suitable match.
Where is the network available?
The current Providence Partners materials identify Metro Detroit, Grand Rapids, and Houston as participating markets.
